If you mean "Ivan was poisoned (by smb)" you`d better say "Ivan BYL otravilEN..." It will be more exactly.
(as I understand, smb put a poison in his meal)

We can say "Ivan otravilsya" if we mean "he ate some bad meal" or "he took poison of his own free will"
