When you feel yourself really sorry. Like when you dropped a brick on someone's foot

I'm not a native speaker, but I don't think you will ever have occasion to utter that expression.

Nope, that expression is used quite often, even in comparatively informal conversations

it's pronounced very quickly, and, imho, 'enhances' your apologies.

Yep. When my neighbour, a big fan of New Year holidays, threw up on my doormat I was ready to kill him, until he said this phrase. It's REALLY effective, if you can add enough sincerity to your voice.

Do phrases like боже мой!, бог знает, боже правый etc constitute blasphemy in Russian? Would Christians get offended?

They might, but the majority certainly doesn't care.

I don't think it will be easy to find someone who would get offended by this.
