Thread: Short message to a medical clinic

I just remembered that I do not know the result of my latest smear which was done in Minsk. The reason is that I didn't stick around long enough to get the results sent to me. They told me to call or email for the results, but I forgot. Nobody at this clinic could speak English, so I need to write in Russian. However a letter like this is too complicated for me right now.

I visited the clinic in June and among other things, I did a smear test for which I did not recieve the results, since I left Minsk shortly after the examination.
Please, to re-assure me, could you respond to this email with information about the outcome of my smear test from June? My date of birth is _______.

Should the surname be in capital letters in Russian letters?
Feel free to correct my writing style - it may not be suitable for Russian even though it sounds fine in English.

I think your surname will be fine put in small letters but starting with a capital one.
