Again, I think you can say both, but in different cases. Like "она шла по этому мосту и смотрела вниз, поэтому не заметила едущий на нее грузовик на огромной скорости". =)

Well, definitely. In the former case these are two separate words. In поэтому - there's just one pronoun.
