I agree it is normally used as a greeting, not really a question. Usually the answer will be "I'm fine, how are you"... and then the real conversation begins. It is just a polite way to say hello and express interest in the other person. That's right. We tend to use this phrase as a kind of "filler", as it were. When someone says this, the person asking the question doesn't expect to be explained the ins and outs of how ill you areThat's not really a question, rather a greeting.
