    I agree it is normally used as a greeting, not really a question. Usually the answer will be "I'm fine, how are you"... and then the real conversation begins. It is just a polite way to say hello and express interest in the other person. Younger people often say "hey! how ya doing?" which means the same thing and does not require an answer on all your ills and troubles...

    Some people use it as a real question- for example when meeting someone you care about and know they have had a bad time, or have been ill.

    That's not really a question, rather a greeting.
    That's right. We tend to use this phrase as a kind of "filler", as it were. When someone says this, the person asking the question doesn't expect to be explained the ins and outs of how ill you are
