Go to the link http://www.tatuagemdaboa.com.br/ and wait a few seconds while it is loading.
Then make your speakers sound a bit lower and fill in two top fields in the menu. You have to write your name in the top one and your second name in the second one. Then just push the VISUALIZAR botton and have fun!

"Nome do amigo zoado" means "name of (your) zoado friend", not "your second name".
No clue what zoado means in Brazillian Portuguese, though. The page also suggests sending a link to the resulting video to your zoado friend

(And, by the way, you can't fulfill blank fields in a form. You can fill them in (or out)).
