Что значит? Действие произошел, собиралось происходит или иначе?

But, essentially, in most cases, it means that the intended purpose of the action has not been achieved. In some cases, it means that the person was going to perform the action, but did not actually do it. In other cases, the action may have been started, but soon aborted. And sometimes the action described by the verb has been actually completed, but its intended result has not been achieved.
