Поняв, что "не раздавив ее, грузовик не уедет", она показала Кубыкину кукиш, мол, не раздавишь!
Кубыкин, у которого, по его словам, уже плыли перед глазами круги, увидев в красном круге кукиш, принял его за дорожный знак "Водитель! Освободи проезжую часть!" и выехал на тротуар, освобождая шоссе идиотке.

Yes, it is, except that there is no such sign. It is supposed to be a joke.

It means that it struck the driver that when she was a child she was run over by some vehicle. Usually "переехать" implies death for a victim and it is used here to emphasize the severity of her supposed experience. "Хорошо" is used for the same emphasis. I don't know, maybe you can translate it as "she was well run over".

It is the usual pattern, like this:
давить - додавить - недодавить
платить - доплатить - недоплатить
грузить - догрузить - недогрузить

"до-" means either "to complete the action" or "to add some more action to it"
"недо-" means "to not complete the action".

Besides, "недодавил" sounds a bit humorous here, like "недоперепил", if you know what I mean. Actually, the author could write:

it's colloquial and should remain colloquial in translation. Otherwise it reads as if there are different degrees of being run over, and she was "well run over" (as in well prepared). Can you say that she wasn't run over sufficiently? Or only half run over?

Rules how to use good and well don't really apply here, you have to look for a similar expression. Anyways, I just use my experience, whatever limited it is, and "well run over" sounds strange to me, unless something is well run over a period of time. You knock somebody down or you do not, there are no degrees in that. You can certainly say "run over badly" or "pretty well beat up".
