I will put it this way: I can think of an example where I, personally, would use only "теперь", but I am sure that there are enough people around who could use both.

It wasn't easy to come up with a context where they are not interchangeable but I managed it.
For example, somebody asks you to do something and you say "сейчас", meaning "I will do it immediately"....
