Are you sure about the first form? I can't imagine any examples

Землей Earth ... yep, oops, well i got it from the song между небом и землей... i just forgot to nominitize it earth-земля.

Well, I've for sure heard both между небом и землей and между небом и землёй. Those are the titles of two different songs, btw. I dunno though, maybe it's incorrect.

what case does "between" fall under ? instrumental ? because i have heard между солнцем и луной... i don't know, it just does not really look like an "instrument" of anything. maybe i will go look it up

On a second thought, Tu's привет sounded a lot like привёт to me, so maybe it's just my unrussian ear that mixes up е and ё.

They really are the instrumental forms of the words in question. "Mezhdu" is always +5 (instrumental)

what about "я сошла с ума" shouldn't it be yмой ? what is up with that crazy C ?

"S" can mean "with" ("s toboj" - "with you") in which case it's with the instrumental case. However, it also can mean "from" ("s teh por" - "from that time on") or "from of"/"out of" ("ptica uletela s doma" - "the bird flew away from the house" [meaning it sat on the house and flew away from that location]) in which case it's with the genetive case.

Ya soshla s uma -> I went out of my mind.

("Na" & "s" are connected, just like "v" and "iz" are connected -> "Ya poehal v Rossiyu. Ya vernulsya iz Rossii." "Ya sizhu na divane. Ya vstayu s divana." (Note: the gen. case with "iz" and "s")
