I don't understand why my book says

Где вы отдыхали летом ?

The book translates "Where did you go for your summer holidays"

I would have said

Где вы отдохнули летом ?

I mean , the action is finished so it should be perfective

Both forms are acceptable

1. Где вы отдохнули летом.
Отдыхали, отдыхали, отдыхали и отдохнули.

2. Где вы отдыхали летом.
Отдыхали, отдыхали, отдыхали и не отдохнули.

I personally find (1) weird, and would be hard pressed to invent a context in which it would sound natural...

Я бы не спросила "Где вы отдохнули летом?", но могла бы спросить "Как вы отдохнули этим летом? Куда-то ездили?".
"Где вы отдыхали летом?" - звучит нормально.

Yes, these sound totally fine... i guess this is because this question is really about the result rather than the process...

Устоявшиеся фразы:
"Где вы отдыхали?"
и
"Как вы отдохнули?"
