В полдень, когда телега остановилась в Телибееве, Кузьма пошел в кабак. Часа два отдыхал Ефрем, а он всё не выходил из кабака.

Why is it выходил and not вышел? When he went into the tavern it's unidirectional (вышел) but the fact that he hasn't come out is expressed with the multidirectional выходил. Is it because a round trip (going in and out) is implied?

I don't know about "directionality" of these verbs. I'd say it's like indefinite against perfect. Compare: "Он не вышел через два часа." - "He hasn't come out after two hours." and "Он не выходил два часа." - "He didn't come out during two hours". In your example "всё не выходил" - "didn't come out during all that time", or even "was not coming out all that time" . Кузьма не выходил пока Ефрем отдыхал.

It's not that Kuzma hasn’t come out but he hasn’t been coming out for two hours while Efrem was waiting for him all that time expecting him to come out any minute.
