"От скольки до скольки?" - If the period is within one day and is measured in hours. (from 8:00am till 6:00pm)
"От которого до которого числа?"- If the period is measured in days. (from May 15 till June 22)

Those are colloquial phrases. If you need official style, it should be rephrased like:
"Укажите период <чего-либо>."
