Is there any difference in meaning between these two words, and when they can be used, or are they completely interchangeable?

To be taken with a grain of salt, the following is my interpretation of what I've found out so far, I'm not a native speaker.

No, they are not completely interchangeable. "Сей" used to be an old demonstrative pronoun for what today is этот, which still exists in some phrases such as до сих пор and as a component of some words. Сейчас means literally "this hour". It can only be used to imply the present or "in a minute". There is a correspondent word тотчас for "that hour", meaning any specific point in time which is not now.

Теперь on the other hand mean "nowadays", "at present". I suppose you can often replace сейчас with теперь, but less frequently the other way round.

I'm not a native speaker either, and what's more, I've had the same question for a long time.

All I can say is that I've heard Дмитрий Хворостовский treat those two words like they are identical. In one concert he says теперь, in another performance сейчас.

All I can add is that I have frequently been corrected when I used сейчас instead of теперь, but I do not recall having been corrected the other way round.

'Который сейчас час?', не 'который теперь час?'

Maybe a time traveler would say 'который теперь час?' I don't know.

Теперь is often implies the idea of consequence while сейчас do not. Теперь often can be translated as "after all", "from that time", "from now on".

- Вчера споткнулся, выходя из туалета. Теперь знаю, что значит выражение "убиться об стену".

"Теперь" is used when something has changed or is going to change. As it-ogo has said it is often used when we tell about consequences of some events. It is even possible that it doesn't mean "at present" or "nowadays". For example "ты получил двойку и теперь не поедешь в Диснейленд". It doesn't mean that they were to go there immediately. The travel could be planned for the next month. It is implied that the plans has changed now.

"Сейчас" on the other hand means just "at present". "Сейчас мы не пойдём гулять, сначала выучи уроки". You shouldn't use "теперь" in such contexts. It can sound old-fashioned.

Not necessarily would, but yes, he could, if an hour of the day always changes when he jumps through time, and he expects it to be different again. He would have to have a companion to make this question sound naturally, though. A passerby from another era would give him a strange look.
That's the primary distinction of "теперь" - implying that something changes.
Unfortunately, "сейчас" is often used in the same sense and things are muddled up.


One meaning of "сейчас" for which "теперь" can never be used:

I think my post is not going to be the ultimate explanation but "теперь" is almost equal to "by now" and "сейчас" equals to "right now". "Теперь" is not so exact, while "сейчас" refers to what is happening right now.

Теперь - используется, когда сообщается какое-нибудь событие, которое наступило после другого события*. Теперь - это наречие, но оно очень близко к значению союза, который соединяет два предложения.

Т.е. после того, как он вчера споткнулся, выходя из туалета, теперь он знает, что значит выражение "убиться об стену".

Сейчас - используется, когда сообщается какое-нибудь событие, которое наступило или происходит непосредственно сейчас. Т.е. это действие не связано с другими действиями.
Например: "Сейчас я живу один". То, что "Сейчас я живу один" не является следствием чего-то.

*другое событие - может лишь подразумеваться и не сообщаться.

Сравните:
Сейчас я живу один. и Теперь я живу один.

Сейчас я живу один.
Я просто сообщаю информацию. Собеседник скажет: "Вау, круто!"

Теперь я живу один.
Я сообщаю следствие/результат другого события (или других событий), о котором говорится или не говорится. Т.е. теперь я живу один, потому что выгнал всех, с кем жил. "Выгнал всех, с кем жил". = другое событие.
Можно сказать: я выгнал всех, с кем жил. Теперь я живу один.
А можно сказать:теперь я живу один. Но тогда собеседник может спросить: "А почему?" и тогда тот ответит: "(Потому что) я выгнал всех, с кем жил".
