Translation of order form
With google Translate and my tiny knowledge of Russian I was able to figure out most of what I needed, but a couple things I can't figure out. Here is the order form http://members.cox.net/gingerale/ozon.jpg
The first one is Country
the second index? I don't know what this means. Zip code maybe?
Third is State
Fourth google translate says is town
Fifth is Address
Sixth google translate says is "Name receiver (specify complete, for example: Ivan Ivanov)" Does this mean the person who will sign for it or something? Or just the person it's being sent to? I'm use to имя or зовут so don't understand anything on this line.
What is эакаэа?

I doubt about the Zip code,
but when you write the British adress the index seems like

It's the person it's sent to, i.e. you.
