2. Police certificate:
Police certificates in all names as well as all dates of birth ever used.
Police certificates must contain references to each place (subject to the jurisdiction of the Russian Federation) in which you live or lived for more than six months since attaining the age of 16.
If the certificates are in russian, you need to have them translated into English.
I said in my application that you changed your family name from XXXX to YYYY. So you need to make sure that the old family name is translated as XXXX.
You can obtain the police certificate from the central police station in Vladivostok.
You also need to collect the police or court paper to prove your family name change.
3. Original birth certificate, photocopy and translation: You only need one for the interview but please make four birth certificates. You will need the rest in the future.
4. Five color full-face photos: 50x50 mm for visa. The eye level should be between 28mm and 35mm from the bottom of the photo. The photos must be of white background.
