I think it might be Polish, given the spelling with a "w" rather than "v".
It depends,
In the past Ukrainian names were transliterated according to a Polish style transliteration, so for example someone with the surname Шевченко would be written Szewczenko. For example when Ukrainians came to Britain after WWII, their names were transliterated this way into 'English'.
Since the name is Ukrainian it would be better transliterated as Polywoda / Polyvoda, since standard translit. of Ukrainian и is y. Ukrainian i is transliterated as i in English.
