That's a very colloquial word and it means more like a screw up rather than a simple mistake.
A bit more:
There is a verb derived from it - косячить/накосячить (imperfective/perfective)
You can also call a person косяк, meaning that that person косячит a lot ;)
Actually you can't call someone косяк. This doesn't sound right to me at all. You can say "он косячит" instead.
