"Заявка" or "заявочка" (diminutive) means something like a "call" here, usually it's a request from the victim, but in this case it's a call from the ambulance. The cop on duty accepted the call and wrote it down (possible into a "registry" or something like that). So, information for cops about the specific incident is called as "заявка".

The cop on duty offers Kirill "заявочку", but Kirill says that he doesn't work with it (at the moment). Then the cop on duty raises an objection to - "you are (was) in reserve".
