first of all I don't think that the word вопросник is commonly used but I'm going to assume that it is something similar to опросник
опросник is something close to survey, when you have a bunch of questions and you just give answers, free answers or chose from the list of answers
анкета means some kind of form where you provide some information, usually personal information, definitely not to pick an answer from a list of answers

These two words may mean the same thing, I guess.
