Oficially (in technospeak) it's "лампа накаливания" (incandescent lamp?). In everyday life it's always "лампочка" (and almost never "лампа").
Light bulb, round small standard = лампочка (you may add "обычная", I guess...)

to change a light bulb = заменить/сменить лампочку; вкрутить новую лампочку
To burn out = перегореть (e.g. лампочка перегорела)
to burn out (meaning you just turned on the light and you saw a spark (искра) = ? I have no idea, I don't see a difference. The bulb burnt out anyway, didn't it? :)

My dictionary says:
мало того — moreover, more than that, not only that
мало того, что — it is not enough that
