Hmm.. what I remember from my school days is a term "a common gender" applicable to the professions. In those cases, the noun itself is masculine while the rest of the related words are in a proper gender.
So, I think the following holds true:
мой врач закончила осмотр. (Женщина закончила.) - past
Я заметил, что
мой врач что-то долго и подозрительно пишет. (Женщина пишет.) - present
Я не думаю, что
мой врач обратит внимание на такие пустяки. (Женщина обратит внимание.) - future
