Доктор/врач both for male and female doctor

Remember to use доктор as an address/title, and врач as a generic noun.

I have heard врачиха often for a female doctor, but only when referring to a female doctor out of earshot. Is it slightly impolite?

It is, but it's mostly vernacular form, that is belonging to slightly informal and uneducated kind of speech.

Все верно. То же самое относится и к слову "докторша"
